Leaving a correctional facility - whether it’s a youth justice centre, prison, or DCJ care - can be complicated. There’s a lot of paperwork, a lot of complicated wording, and not much explaining happening.


We work with DCJ, youth justice, and adult corrections regularly, so we know how to bridge the gap between those systems and your NDIS supports. We’ll explain everything to you in plain English and make sure nothing falls through the cracks,


The Right Support For the Right Reasons


We’re careful about choosing people who understand the complexities of re-entering the community, especially for young people. We can help you to find medium-term accommodation (MTA) in a place that is familiar to you.


Many young people are in DCJ because their families simply don’t have the capacity to support them right now. We keep that in mind and make sure our approach is built around each person - not assumptions.

Bridging the Gap Between Jail and Home

At Golden Heart, we understand the gaps between jail and a home.

We work with participants who have no core funding and require a place.


We work directly with your allied health team, correctional facility officer, and behaviour practitioners to secure you a house while collecting the evidence to support your review.

Trauma-Informed Rehabilitation Focus

Our support workers are trained on a DBT basis to ensure comprehensive practice when dealing with complex participants.


We focus on rehabilitation, not incarceration.
